Selection Criteria

Nomination Form

All selections must be current students at LCMS and in good standing. Including, but not limited to:

• Absolutely no failing or incomplete grades for T1, S1, S2, or T3. 
• Regular attendance, as determined by the Wall of Fame Committee (90%) 

Criteria: 

1. State Competition Qualifiers (when qualification is based on more than mere entry – i.e. state track)
2.  Qualifiers for Activities above the local level (district (not LC-district, but district), regional or state) includes fine arts, or other activities approved by the committee 
3. Principal’s Honor Roll (4.0 GPA for 1st or 2nd semester) 
4. Titan Honor Roll
   1. No more than 2 unexcused tardies per semester
   2. No documented office referalls for 1st or 2nd semester 
   3. No failing or incomplete grades 
5. "Rising Star" Student (2 per grade level) submitted by the team 
6. Parental Notification for outstanding service or level of involvement in non-school activities above the local level
7. Staff nominations for outstanding accomplishments 
   1. Undefeated athletic season (Undefeated Team / Individual). Must be nominated by the coach. 
   2. Breaking a school record. Must be nominated by the coach. 
   3. Other accomplishments approved by the committee. 
8. Optimist Most Outstanding Student Winners (2 per grade level) 

Students who feel that they have not met the established criteria due to extenuating circumstances may appeal to the Wall of Fame committee by May 9th by emailing Dr. Reynolds (mandie.reynolds@lewiscentral.org) . She will forward it on to the committee. 

Selection of students to be recognized on the “Wall” will be limited to those nominated using the above criteria.  The nomination forms will be reviewed by the selection committee and approved by a majority vote of that committee.

The selection committee will consist of a diverse segment of middle school staff and the building principal.  The committee will make the final decisions regarding selection for the Wall of Fame!

Students may be nominated throughout the school year.  All nominations for the current school year CLOSE ON FRIDAY, MAY 9TH!  Students nominated for accomplishments occurring after that date will be considered for approval the following year. 

Please fill out the Google Form by clicking on the link above.  YOU MUST FILL OUT A DIFFERENT FORM FOR EACH STUDENT NOMINATED (Ex. If you are nominating a team for an undefeated season you can’t fill out one form for the team.  You have to fill out a form for each athlete on that team).
