Immunization Record

Iowa law requires that every student in the Lewis Central Community School District have certain immunizations prior to school enrollment. If we do not have the proper certificate on file for your child prior to the first day enrolled, we have no choice but to deny him/her from enrolling. In most cases, all former students have this certificate on file with the school. However, all new students wishing to enroll must have such certification of immunization as well as any transfer students.
