Is Your Child Receiving Medical Treatment for Asthma or A Serious Allergy?
Asthma/Anaphylaxis Action Plan Required

Welcome Back! We have many students at Lewis Central with
asthma or anaphylaxis (a severe allergic reaction). If your son or
daughter is currently using an inhaler or requires an epi-pen, we will
need an Asthma/ Anaphylaxis Action Plan form completed by their medical
provider. This form is available on the "Health Forms" link on the left side of this page.
We would greatly appreciate your cooperation in having this form
completed and returning it to school as soon as possible. An emergency
care plan will be developed for those students who have an Asthma Action
Plan directive on file. In the event your child has a severe asthma
attack or allergic reaction and we do not have an Asthma Action plan on
file, our district wide protocol will be initiated. This will involve an
emergency squad call and transport at the parent’s expense. A student’s
health and safety is our top priority. Feel free to contact one of the
district nurses with any concerns or questions.
